How do I create a custom form using Forms LT 1.5 Lite CRM?
Login to into your Joomla 1.5 Admin control panel. Click on components link in the top navigation and go to SerrBizFLT.

The FORMS control panel opens.

Click the FORMS icon.

We will begin on the LEFT side of this page, then after discuss the RIGHT hand side.
Please note, when started, that the ADD FORM option is selected automatically. If you already have a form that needs to be edited, you would select EDIT check box, then the form.

To create a new form follow the steps below.
1) In the CC email field, plug in the email address of whom else, besides the default administrator, should receive a copy of the FORM data.
2) In the FORM NAME bar create an intuitive name that coincides with the content name article. It will be a lot easier to remember what the form is for.
3) Choose a START PUBLISHING DATE, and an END date. If there is no end date, you can just leave blank.

Moving on…
4) You have an option to turn on the EMAIL CONFORMATION. This means the USER will NOT see your final message such as a coupon code, or download link, until they VALIDATE their email address. This helps reduce spam.
5) Choose whether to ADD AS USERS or not. If selected, the USER will be added as JOOMLA USER. * This is REQUIRED if you are using the LITE CRM.
6) Check DETAILS OF CONSTANT CONTACT if you want user data to be added to your Constant Contact news letters. *CONSTANT CONTACT API information must be pre-configured in the Forms LT control panel.
7) The people in the ASSIGN LEADS TO field are pre-set in the configuration panel. If there isn’t anyone listed it will assign by default to ALL. You must assign the leads to a specific person if you do not want the default option.
8) Check USE VM only if this form is going to be used to replace the “ask a question” function of Virtuemart. * Requires custom modifications.
9) Choose ACCESS LEVEL to whom this form should be available to. You have the option to make this form available to the public, registered users only and front end administrators.

10) If you’ve already created custom fields AVAILABLE CUSTOM FIELDS will make it possible to use them on this form… Select the pre-made custom fields you wish to use in the form you are making. *By default two HIDDEN fields are available: Product URL and Product ID. These fields can ONLY be used if the form is being used as the replacement for the ASK A QUESTION function in Virtuemart. Be sure to click the USE VM checkbox.
11) Custom SEF: In Joomla 1.0, Forms LT allowed you to set a custom SEF for the REGISTRATION COMPLETE page. This is the page people will see after COMPLETING the form. We made this OPTIONAL so you can integrate Google Analytics Goal Conversion Tracking. *Beta – In Forms LT J 1.5, we have NOT tested this using sh404, so it may NOT work. 12) Add a MESSAGE you want to show the user AFTER the user/customer fills out the form. Here you can say anything you like, and you can use HTML for linking to downloads, etc.
13) Use 3rd Party Tracking Code: This option lets you ADD special code such as Google Conversion Tracking, Yahoo Search Marketing, etc.

Before we finish let us go to the RIGHT side of the screen that was introduced in the beginning of this tutorial.
14) Over to right you can choose which FORM FIELDS you want to use. It is the information you require from the user that will be filling out your form. You can make any field optional or required. If required, the USER will be notified they cannot submit the form until all fields are complete.

15) Now, click save.

And you are done!
